They are looking to add to their North or Northwest clinic as they continue to develop a name for themselves. You will be reporting to the Head of Allied Health and work in a mix of clinic and community-based work , with options to work from home too. You will: Be responsible for the governance of clinical practice guidelines and ensuring compliance with necessary procedures. Educate and support care givers and take charge of care coordination and management, as well as management of KPIs. Skill develop in self-care, self-management, home management and community integration.
